Against the Odds: Rapid Evacuation from Kota Kinabalu to Singapore

International SOS coordinated a specialized medical evacuation for a senior executive following a mountain biking accident in Kota Kinabalu, Sabah, Malaysia, ensuring a safe return to Singapore within 72 hours.

Situation: A Logistical and Medical Standstill

While on a personal trip in Kota Kinabalu, Maarten, a senior executive, was involved in a mountain biking accident. He was admitted to a local hospital with a fractured rib and a punctured lung.

While his condition was stable, the nature of his injury presented a significant hurdle for travel. Due to how air pressure changes with altitude, there was a risk that the lungs would collapse further during a flight. The advice was for Maarten to avoid flying for at least 30 days.

The Challenge: The Gap in Standard Coverage

Maarten needed to return to work promptly but his primary insurance provider was unable to facilitate an immediate solution. Their assessment concluded that he was medically unfit for commercial travel, and the high cost of a specialized private air ambulance, which can fly at lower altitudes to maintain sea-level pressure, was not a viable option under his policy.

Faced with the prospect of a month-long recovery in a regional hospital far from his home, office, and family, Maarten required a more proactive medical and logistical intervention

The Solution: Expert Assessment and Integrated Care

Seeking a second opinion and a faster resolution, Maarten’s office engaged International SOS. Our approach moved beyond standard insurance protocols, focusing on a medically led strategy to get him home safely and efficiently.

1. On-Site Medical Evaluation (24–48 Hours)

Within two days, International SOS deployed a Medical Advisor to the hospital in Kota Kinabalu. This allowed for a direct, face-to-face assessment of Maarten’s clinical status. By reviewing fresh X-rays and consulting with the local treating physicians, our team determined that Maarten could indeed fly sooner than the 30-day window, provided specific medical safeguards were in place.

2. Specialized Commercial Evacuation Plan

Instead of waiting for the 30-day mark or relying on a costly low-flying private jet, we developed a plan for a medicalized commercial flight. This included:

  • Medical Escort: An International SOS medical professional accompanied Maarten throughout the journey.
  • Emergency Equipment: The escort carried specialized equipment on board to manage potential changes in lung pressure during the flight.
  • Comprehensive Logistics: We arranged for Maarten’s children to return to Singapore early with a chaperone and ensured his wife could travel on the same flight as him.

“The doctor flew down to the hospital in Kota Kinabalu to assess my condition and we were on a flight back to Singapore the next morning. I was very impressed with how quickly everything was arranged.”
— Maarten Kelder, Former Senior Partner at a Global Business Management Consulting Firm

3. Seamless Transition to Singapore (72 Hours)

Upon arrival in Singapore, the transition was seamless. International SOS had already pre-arranged his admission to a local hospital and facilitated the immediate commencement of diagnostics and specialist treatment.

Impact

  • The intervention by International SOS significantly altered the recovery and return-to-work timeline and the overall experience for the client.
  • Timeline Acceleration: Reduced the wait time to return home from 30 days to 72 hours.
  • End-to-End Coordination: Managed all family logistics, including chaperone services for children, removing the administrative burden from the family.
  • Clinical Safety: Successfully managed a high-risk respiratory condition during transit with zero complications.
  • Rapid Return to Work: Due to the early start of specialist treatment in Singapore, Maarten was able to return to his professional duties shortly after discharge, a result that would have been impossible had he remained in Sabah.

“I would score the experience 12 out of 10, basically. I recommend that all organizations use International SOS to support their employees. An experience like that is worth a million.”
— Maarten Kelder, Former Senior Partner at a Global Business Management Consulting Firm

Key Takeaways

  • Beyond Insurance: Standard insurance providers often operate within rigid parameters. A medically led organization can provide the clinical nuance needed to identify safe, alternative solutions for complex cases.
  • Integrated Logistics: True Duty of Care involves more than the patient; supporting the family and managing the "peripheral" logistics is essential for executive wellbeing and peace of mind.
  • Early Intervention: Deploying a medical expert to the site early in the process is the most effective way to shorten recovery timelines and reduce overall costs.

This case illustrates the value of proactive medical management. By combining clinical expertise with sophisticated logistics, International SOS ensured that a serious injury resulted in a swift recovery rather than a prolonged period of displacement.
